Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident Work closely with Case Management to coordinate patient discharges QUALIFICATIONS REQUIREMENTS:
Must possess at minimum a High School Diploma or equivalent G.

D Social Work License preferred, not required Must have experience in long term care or skilled nursing facility
