Nurse Practitioner (GAP) Community Based Care Service

The GAP Advanced Nurse Practitioner (ANP) provides a wide range of health services to eligible Veteran patients who present with urgent, acute, acute on chronic, and chronic medical conditions. The ANP is knowledgeable in:
the assessment, treatment and evaluation of a comprehensive range of health care problems; emergency measures; preventive health; prescribing medications and other treatments; ordering and interpreting laboratory, diagnostic studies and procedures; and patient and family health education. The GAP ANP will primarily serve as a Provider of the Day (POD) for the empaneled Patient Aligned Care Teams (PACT) at their assigned site, assisting with any situation in which a patient has a healthcare need that cannot be accommodated timely by his/her Primary Care Provider (PCP). Such assistance may occur via face to face, secure message, telephone, or video telehealth encounter. While the GAP provider does not typically maintain a patient panel, depending on staffing deficiencies/concerns, the GAP ANP may be required to assume a Primary Care panel while provider recruitment/hiring is in process. As below there may be a limited need for occasional travel between neighboring CBOCs. Duties include but are not limited to:
Investigates health problems or health needs by gathering and assessing historical information; Performs a physical examination appropriate to the patient's history; Diagnoses health problems based on interpretation of the patient's history, physical exam findings and diagnostic test results; Develops treatment plan tailored to the patient's needs, which includes health education, pharmacologic and non-pharmacologic treatment; Prescribes medications as appropriate; Makes referrals for specialty evaluation or treatment; Enters progress notes in the computerized medical record and completes workload documentation in a timely manner; Providers timely follow up on test results, timely completion of view alerts and timely response to secure messages as per policy; Collaborates with the PACT members as needed to ensure accessible, comprehensive, coordinated, continuous care for all assigned Veterans; Able to work in a technologically advanced environment utilizing state of the art alternative methods of patient care such as video telehealth; Provides quality care that meets the professional standards and works within his/her privileges at all times. Performs other related duties as assigned. The incumbent may be required to transport objects (i.e. equipment, mail), therefore he/she must possess a current, valid driver's license. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Nurse Practitioners and Clinical Nurse Specialists. On and after March 17, 2009 , registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before March 17, 2009 , who obtain such qualifications on or after March 17, 2009. Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Condition of Employment:

Nurse II - Master's degree in nursing or related field with BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. Nurse III must demonstrate performance and leadership that is broad enough to improve the care for a group of patients. The Nurse III is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, VISN, or VACO. In order to be considered for appointment or promotion to Nurse III, outcomes must be documented. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. Columbia VA Health Care System is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, which admissions for medical co-morbidities. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and object may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
